If images or format of Health Report DO NOT show up corrrectly in your email, please ensure that you have your email program set to allow HTML and/or images to display.
Start by entering the traveller's details and an email address to send the completed report to. You must select a Reason for Travel from the drop down box.
When entering travel details, you select the countries to be visited from the drop down boxes, the date of arrival in each country and the date of departure from each country. The date of departure is usually the same as the date of arrival in the next country. The date of departure from the final country is assumed to be the date of return to the country from which your travel started.
When entering dates, use the drop down calendar that appears when you click on the date box. If the date is selected incorrectly, either delete it and re-enter the date, or just click on it and change it manually.
If you enter the date manually, please use the format dd/mm/yyyy e.g. 12/01/2010 for 12 Jan, 2010. If you enter the dates incorrectly, it will only affect the calculation of the trip 'duration'. defaulting to '1 day'. Everything else will print correctly.
If you make a mistake or want to change details for a country that you have selected, you can click on the 'X' at the right side of the row to delete the country, then you can resume the health report.
If you want to cancel more than 1 country, just click on the 'X' at the right side of each country you wish to delete.
When you click on the "Prepare Report" button, your health report will be shown. Your health report will also be emailed to the email address that you specified. Please ensure that you have your email program set to allow HTML and/or images to display.
